2.1 CONSENT

Upon receipt of this consent, you agree that we may collect, use and disclose your information as specified below:

Consent to provide your personal information to a credit reporting body (“”) – you authorise us and each Lender () to obtain a report or information (including a credit report)about your, and each associated individual’s (such as a director and guarantor) consumer and commercial credit worthiness from a CRB, we and each Lender may discloser personal information such as your name, date of birth, and address to the CRB to obtain an assessment of whether that personal information matches information held by it.

UPON RECIEPT of this Privacy Disclosure Statement and Declaration, you appoint myFinancex as your agent to obtain credit report on your behalf:

You acknowledge that you have read and understood this Privacy Disclosure Statement and Declaration and consent to the collection, use, and disclosure of your personal information in accordance with this statement.
You declare that all information that has be provided is truthful and accurate.
myFinancex will reserve the right to withdraw any applications is any information is to be found fraudulent in any way and will not be held responsible for any losses financially or mentally due to this.
Upon receiving notice, we will cooperate to the extent of the law with any of the government bodies with information we have been provided and collected.
